I took my 95 year old mother in Oct 2015 to get a handicap car hanger and SC ID card. I did not have her birth certificate so I attempted to get these items today. I had the forms 447-NC and RG-007A filled out, her doctor signed yesterday and wrote out a prescription for the place card. I had her old MI drivers license (expired 5/18/15) her SS card, her MI handicap card to expire 5/18/16, her birth certificate and a bank stmt to prove she now lives with me. After waiting a half hour in line I was then told I need her marriage license to prove name change. I have no idea where to find her marriage license. She was a widow and remarried in 1987 and has been a widow for the past 21 years. This is most difficult for a 95 year old to get out to get this stuff and to yet again have to drag her out a third time is absurd. We did not have it this difficult when she applied for it when she lived in Michigan. Is there any way we can bypass the marriage license. The ID card is just to show for cashing a check when she gets her prescriptions. And the handicap card helps to park close at the store, church etc. Thank you for your help in this matter.
