Specifically looking for form APP-107 which should be a form to subpoena employment records, disciplinary actions, interview with employer as to what they reported to SCDEW, etc. to submit to the appeals tribunal. When calling SCDEW you do not have an option to speak with a representative to answer questions. When trying to visit the web site as noted on the determination of claims, www.dew.sc.gov/appeals.asp, this web address is no longer available. It is required that an appeal be filed within 10 days of mailing of this notice, but the information given to claimants is not available. If you can help us, I would greatly appreciate a response as soon as possible, we have three (3) days left to get this information back to SCDEW. Thank you in advance for your timely response and thank you for serving our state! Kenneth L. & Nadine G. Boylston 67.197.182.183
