The first letter I received from PEBA stated they needed an original or certified death certificate, a copy of my driver's license, a copy of my social security card and an election of Death Benefits form signed and notarized. I made arrangements to collect the above items and sent them in. Then I get a letter stating PEBA needs an Affidavit to Collect. I go to Conway and get this from Probate. They give me only copies with the stamp of Probate Court, the date and the judge's name telling me only DMV requires originals. I send this in. Today, Oct 3, I get a letter with another Election of Death Benefits to be completed for 1/2 the retirement check, signed, witnessed and notarized again and a statement saying they need an original or certified Affidavit to Collect. The first letter did not mention an Affidavit to Collect, the second did not mention it needed to be certified. Why could they not asked for all this at one time? Why could they not state certified or original in the second letter? Now, I have to go back to Probate Court. I feel the stress this has added on top of the stress and grief of my mother's death was absolutely unnecessary. They could have asked for all this at one time and told me it had to be certified. There is NO indication that it had to be a certified or original copy until today. There certainly needs to be an "overhaul" of this process. Thank you.
