My family and I recently moved from SC, and we mailed our two vehicle tags back to your Blythewood office, along with four other forms. Two of the forms were Form #452 (one for each relinquished tag) and two change of address forms - one for my husband, and one for myself. The Post Office tracking number shows that the package was received at the Blythewood DMV on 8/20/16. We had requested a receipt to be mailed to us, for two reasons...one was that we wanted to make 100% sure the DMV office did indeed receive the package and the other reasons was because we want a refund on the amount left remaining on one of the tags. We have heard nothing back from them, so this morning I called the Blythewood office and I spoke with agent #342, whose name is Phillip. (He was very nice and polite, by the way). How frustrating to find out that the forms we sent (both forms #452) are not there! He shows that our address is changed, and he shows that the tags were relinquished, but there is no record of the forms that we filled out to ask for a receipt to get a refund. From what he told me, they were never scanned in to the system. This is SO frustrating. We have done all we were told to do and followed all the rules, and it is just so aggravating that they did not do their job. We are asking if there is any way you can investigate and find out how we can go about getting our refund? The refund is due to us on tag #2673EP (2011 Ford Fusion).
